Permission to skate & player release forms

Permission to Skate form Information:

Any player currently registered with GKGHA who wishes to skate and/or tryout for another association must have a Permission to Skate (PTS) form signed by a designated executive member of GKGHA.  If you need a PTS form, please read and follow the instructions below.  PTS forms are only valid between April 17^th – May 31^st and August 15^th – October 1^st as per OWHA rules.  PTS forms will not be issued prior to April 17^th. 

For Rep players requiring a PTS form, please contact Kevin Geen at kgeen@simplexgrinnell.com.  For House League players requiring a PTS form, please contact Tracy Airhart at airhart@alcdsb.on.ca.

**Please specify your daughter’s name, their current Ice Wolves team and the team/association your daughter intends to skate with in your email.    PTS forms are valid only for the team and the dates recorded on the form.  A separate form is required for each team/association for which the player wishes to try out for.  Please allow 5 business days to process.

Player Releases:

If your daughter is successful in tryouts with another team/association and requires a release from GKGHA, please email Casey Held at gkghaowrep@gmail.com   All fees must be paid and jerseys returned to be released from GKGHA.
